This foundation was established in loving memory of Laiyah Amaria Polk, whose life was profoundly impacted by sickle cell disease. Her strength, resilience, and unwavering spirit continue to inspire our mission to advance research, improve patient care, and support families affected by this disease. While her journey ended too soon at five years old, her legacy lives on through our commitment to raising awareness, investing in innovative treatments, and helping bring us closer to a world where no one loses a loved one to sickle cell disease. Every life we touch and every step toward a cure honors her memory and reflects the hope she carried for future generations.

When we founded the Forever 5 Lap Inc., we did so with one purpose in mind: to ensure that no individual or family affected by sickle cell disease has to face the journey alone.
Sickle cell disease impacts lives every day, but it does not define the strength, resilience, or potential of those living with it. We believe that through education, advocacy, compassionate support, and community partnership, we can create lasting change for patients and their families.
Every program we develop, every event we host, and every life we touch brings us one step closer to a future where sickle cell disease receives the attention, understanding, and investment it deserves.
